New Player 2 Practice Trial Registration (Fall Ball 2022)

West Des Moines Lacrosse Club welcomes new players to trial 2 practices at no cost. Stickes will be provided. Please complete the form below prior to attending practice.

GIRLS

How did you hear about WDM Lacrosse Fall Ball?

Waiver

I consent to letting my child participate in up to two (2) West Des Moines Lacrosse Club (WDMLAX) practices so my child may evaluate their level of interest before formally signing up and paying US Lacrosse membership fees, any league fees, and WDMLAX player fees. I fully understand the inherent risks involved with the sport of lacrosse. I agree to allow WDMLAX officials/coaches and/or facility officials to arrange for emergency medical care for my child in the event that such action is deemed necessary. I hereby save and hold harmless WDMLAX from any and all liability from injuries, incidents, or legal matters arising from or in any manner associated with my/my child’s participation in activities connected to WDMLAX. If my child would like to continue, I agree to complete Fall Ball Season registration.

About US

The West Des Moines Lacrosse Club (WDMLAX) fields boys and girls teams for players in grades 3-12. We offer programs for youth, middle school and high school. The club is an all-volunteer, non-profit 501c3 organization, and strives to provide players with the best coaching, facilities, and competition available. WDMLAX is a member of several leagues based on gender and age of the players. We welcome players from various communities and schools in central Iowa. Regardless of whether you've played the game before, you are welcome to join our club or join us for a practice to see if you like the sport. We are not affliated nor sponsored by a school district, however, we utlize West Des Moines School District facilities and thank them for their support to grow the sport of lacrosse in Iowa.
